Oestrogen and Anti-Androgen Therapy: Expected Timeline

Feminising hormone therapy typically involves a combination of oestrogen to promote feminine secondary sex characteristics and an anti-androgen or gonadotropin-releasing hormone agonist to suppress endogenous testosterone.

The initial physical shifts usually begin within the first one to three months. Patients often notice a decrease in skin oiliness, softer skin texture, and a reduction in spontaneous nighttime erections. Muscle mass and strength begin a gradual decline, while fat distribution slowly starts shifting toward a more gynoid pattern, concentrating around the hips, thighs, and buttocks.

Breast development generally initiates between three and six months. It begins with subareolar breast buds, which can be tender to the touch, and progresses slowly over two to five years. Breast size and shape vary considerably based on genetics and body composition, mirroring the natural variations seen in cisgender women. Redistribution of body fat toward an android pattern around the abdomen, alongside increases in muscle mass and upper body strength, becomes noticeable within six to twelve months. Scalp hair thinning or androgenetic alopecia may also develop over time, depending on genetic predisposition.

  • Clitoral growth or facial bone structure changes that occur if hormones are taken during adolescence
  • Male-pattern scalp hair loss triggered by dihydrotestosterone
