For transgender and non-binary individuals, as well as intersex people undergoing medical treatments, decisions around preserving biological reproductive options must often be made prior to starting gender-affirming therapies. Hormone replacement therapy (HRT) and gonadal surgeries can alter, reduce, or permanently end biological fertility. Navigating these choices requires a clear understanding of the medical pathways, timing, financial costs, and ethical considerations involved.

Access to reproductive healthcare is a human right, yet systemic barriers often prevent gender-diverse individuals from receiving timely counselling and care. Understanding the options available today allows individuals, families, and healthcare providers to make informed choices that align with long-term family-building goals.

Medical Impacts of Gender-Affirming Care on Fertility

Gender-affirming medical interventions affect fertility in distinct ways depending on the treatments received and individual physiology.

Exogenous testosterone therapy typically suppresses ovulation and alters ovarian tissue function over time. While some transgender men and non-binary people achieve pregnancy after pausing testosterone, long-term outcomes vary, and resumption of ovulation is not guaranteed. Oophorectomy, the surgical removal of the ovaries, results in permanent loss of biological fertility.

For transgender women and non-binary people assigned male at birth, estrogen and androgen-deprivation therapies impair spermatogenesis. Sperm quality and count decrease significantly during hormone therapy. While stopping hormones can sometimes restore sperm production, recovery is unpredictable and may take months or years. Bilateral orchiectomy, the surgical removal of the testes, permanently terminates sperm production.

For intersex individuals, fertility options depend heavily on underlying genetic and physiological variations, as well as whether surgical or hormonal interventions occurred during childhood or adolescence. Medical guidelines increasingly emphasize early, comprehensive discussions before any irreversible medical procedures take place.

Fertility Preservation Options for Transgender Men and AFAB Individuals

Transgender men and individuals assigned female at birth (AFAB) have several established and emerging options for preserving genetic material.

  • Oocyte Cryopreservation (Egg Freezing): This process involves ovarian stimulation using hormonal medications over two to three weeks, followed by an ultrasound-guided retrieval procedure. The retrieved mature eggs are frozen for future use through in vitro fertilisation (IVF).
  • Embryo Cryopreservation: If a partner or donor sperm is available at the time of retrieval, eggs can be fertilised immediately to create embryos, which generally have higher survival rates during freezing and thawing than unfertilised eggs.
  • Ovarian Tissue Cryopreservation: This experimental yet rapidly advancing technique involves surgically removing and freezing ovarian tissue. It is primarily considered for prepubertal youth or individuals who cannot undergo hormonal stimulation due to timing or dysphoria.

Undergoing ovarian stimulation often requires temporary cessation of testosterone therapy and may induce physical symptoms, such as breast tenderness or changes in mood, which can heighten gender dysphoria. Compassionate, gender-affirming clinic environments are vital to supporting patients through this process.

Fertility Preservation Options for Transgender Women and AMAB Individuals

For transgender women and individuals assigned male at birth (AMAB), options primarily revolve around sperm collection and storage.

  • Sperm Cryopreservation (Sperm Freezing): This is the most straightforward and established method. It involves providing one or more semen samples via masturbation, which are then analyzed, frozen, and stored in a cryobank.
  • Surgical Sperm Retrieval: For individuals who experience severe dysphoria related to masturbation, or those with low sperm counts due to prior hormone use, procedures such as testicular sperm extraction (TESE) can retrieve sperm directly from testicular tissue.
  • Testicular Tissue Cryopreservation: Currently considered investigational, this method involves freezing testicular tissue prior to puberty or hormone therapy, holding potential for future maturation techniques.

Ideally, sperm freezing occurs before initiating estrogen therapy. If hormones have already been started, providers typically recommend pausing treatment for several weeks or months to allow sperm production to recover, though this period can present psychological challenges due to the temporary return of secondary sex characteristics.

Ethical Considerations and Informed Consent in Clinical Settings

Clinical guidelines from major endocrinology and reproductive medicine societies state that clinicians must discuss fertility preservation with all patients prior to initiating gender-affirming medical treatments. However, implementation remains inconsistent.

Financial Barriers, Insurance, and Legal Frameworks

The cost of fertility preservation presents a significant hurdle for many trans and intersex people. Expenses include initial consultation fees, medication for ovarian stimulation, retrieval procedures, and annual storage fees for frozen gametes or embryos.

  • Insurance Coverage Gaps: Many public healthcare systems and private insurance policies categorize fertility preservation as an elective procedure, excluding it from standard gender-affirming care coverage.
  • Mandated Coverage Progress: A growing number of jurisdictions are passing "iatrogenic infertility" mandates, which require insurers to cover fertility preservation when medical treatments, including gender-affirming therapies or surgeries, carry a risk of causing infertility.
  • Legal Parenthood Risks: Legal recognition of trans parents varies globally. In some regions, a transgender man who gives birth may be misgendered on birth certificates, or a non-gestational partner may face complex adoption processes to establish legal parental rights.

Navigating these financial and legal landscapes requires careful planning and, often, consultation with legal experts specializing in LGBTQ+ family law.

Family Building Beyond Genetic Preservation

Preserving gametes is not the only path to building a family. Transgender and intersex individuals utilize diverse strategies to achieve parenthood, reflecting a range of personal preferences, financial realities, and medical circumstances.

  • Donor Gametes and Surrogacy: Using donor eggs, donor sperm, or a gestational carrier allows individuals or couples to build families when genetic preservation was not possible or desired.
  • Adoption and Fostering: Many trans and intersex adults choose adoption or foster care. While discriminatory barriers still exist in certain regions, advocacy organisations actively work to expand pathways for prospective LGBTQ+ parents.
  • Co-Parenting and Community Families: Non-traditional family structures, including co-parenting agreements between friends or multi-parent households, offer supportive environments for raising children outside standard legal frameworks.

Every pathway to family building carries value, and choosing not to pursue biological parenthood is an equally valid decision for many trans and intersex people.

Making decisions about fertility preservation involves balancing personal identity, medical timing, and long-term life aspirations. Working with a multidisciplinary care team, including endocrinologists, reproductive specialists, and mental health professionals, ensures that individuals receive comprehensive guidance tailored to their unique circumstances.

Clear communication, access to accurate clinical information, and self-advocacy are essential tools when navigating health systems that are still adapting to the needs of gender-diverse patients.
