Dilation is medically necessary to maintain both the depth and width of the vaginal canal as scar tissue contracts during healing. The initial schedule is intensive, usually requiring three to four sessions daily, lasting twenty to thirty minutes each, during the first few months. Over the course of a year, the frequency gradually decreases.

  • Phase 1 (Months 1-3): Intensive schedule, typically 3 to 4 times daily, focusing on depth retention and tissue pliability.
  • Phase 2 (Months 4-6): Moderate schedule, usually 2 times daily, integrating pelvic floor muscle relaxation techniques.
  • Phase 3 (Months 7-12): Maintenance schedule, dropping to 1 time daily or less, depending on clinical guidance and sexual activity.

Dilation can be physically uncomfortable and emotionally challenging. Pelvic floor physical therapy plays a crucial role in helping patients manage involuntary muscle spasms, elevated pelvic floor tone, and discomfort during dilation or sexual activity.

Complications and Clinical Interventions

While serious complications are uncommon when surgeries are performed by experienced teams, patients must be trained to recognize early warning signs that require urgent clinical evaluation.

  • Tissue Necrosis: Darkened, cold, or unresponsive tissue, caused by compromised blood supply, which requires immediate surgical assessment.
  • Hematoma and Seroma: Excessive swelling or fluid pooling under the skin, often needing bedside drainage or compression adjustments.
