Preventative medicine relies on clear, evidence-based guidelines. However, traditional screening protocols are almost universally designed around a binary model of biological sex, assuming a fixed relationship between an individual's legal sex marker, their chromosomal profile, and their anatomical organs. For transgender and intersex individuals, this assumption breaks down. Effective preventative care requires a nuanced understanding of an individual's current anatomy, past surgical history, and hormone exposure, regardless of what appears on their identity documents or medical chart labels.
Understanding how to navigate these screenings helps patients advocate for their health and enables clinicians to deliver accurate, respectful, and life-saving care.
Anatomy-Based Screening vs Gender Labels
For decades, preventative care reminder systems have relied on legal or administrative gender markers. Automated EHR systems routinely generate alerts for cervical screening based on a female marker, or prostate screening based on a male marker. This administrative reliance creates significant clinical gaps. Transgender men who retain a cervix may miss crucial cervical screening reminders if their administrative marker is changed to male. Conversely, transgender women may receive irrelevant screening prompts while missing necessary prostate health evaluations.
Clinical advocacy organisations increasingly recommend an anatomy-based approach to preventative screening. Under this framework, screening recommendations depend entirely on the presence of specific organs, irrespective of gender identity, legal marker, or physical appearance. A person with a cervix requires cervical cancer monitoring; a person with prostate tissue requires prostate health surveillance.
Implementing anatomy-based screening requires clear, confidential documentation within medical records. Modern clinical protocols encourage providers to maintain an up-to-date organ inventory. This inventory tracks surgical history, including procedures such as hysterectomy, oophorectomy, orchiectomy, or vaginoplasty, ensuring that appropriate screening prompts are generated accurately without exposing patients to administrative misgendering.
Breast and Chest Cancer Surveillance
Breast cancer screening guidelines for transgender and intersex individuals are guided by duration of hormone exposure and tissue composition. Risk is determined by the presence of glandular breast tissue and the cumulative length of time that tissue has been exposed to estrogen.
For transgender women and non-binary individuals on feminising hormone therapy, prolonged exposure to estrogen and progesterone stimulates the development of dense fibroglandular breast tissue. Clinical guidelines generally recommend that transgender women who have used feminising hormone therapy for five continuous years or more initiate mammography screening at age 50, aligning with screening intervals for cisgender women. Individuals with additional risk factors, such as a strong family history of breast cancer or known BRCA gene mutations, may require earlier or more frequent imaging.
Screening protocols must reflect biological reality rather than administrative categories. If an organ is present, it requires appropriate clinical monitoring.
For transgender men and non-binary individuals who have not undergone chest masculinisation surgery (top surgery), breast cancer screening follows standard guidelines based on age and family history. For those who have had chest masculinisation, the remaining risk depends on the surgical technique used. While top surgery removes the vast majority of breast tissue, small amounts of subcutaneous tissue often remain. Although the overall risk of developing breast cancer is dramatically reduced, routine clinical chest exams remain an important part of preventative care, and any new lumps or skin changes require immediate evaluation.
Intersex individuals face variable risks depending on their specific genetic profile, gonadal status, and hormone usage. For example, individuals with androgen insensitivity syndrome who retain endogenous estrogen production or take supplemental estrogen follow recommendations similar to those for cisgender women, tailored to their individual medical histories.
Cervical and Reproductive Tract Screening
Cervical cancer prevention relies on routine screening for high-risk human papillomavirus (HPV) strains and cytological evaluation via Pap smears. Anyone who has a cervix, regardless of gender identity or hormone use, requires regular cervical screening according to standard national guidelines.
Transgender men and non-binary people with a cervix often face unique barriers to cervical screening. Long-term testosterone therapy can induce vaginal and cervical atrophy, making speculum examinations uncomfortable or painful. Atrophy can also lead to an increased rate of unsatisfactory cytological samples, requiring repeat testing.
To improve care, clinicians can use smaller speculums, apply topical estrogen cream for a short period before the exam, or utilise topical local anesthetics. Furthermore, self-collected HPV sampling is becoming widely accepted across many healthcare systems. Self-sampling allows individuals to collect their own vaginal swab in a private setting, offering a reliable, highly acceptable alternative that significantly reduces distress while maintaining diagnostic accuracy.
For individuals who have undergone a total hysterectomy including removal of the cervix, routine cervical screening is generally discontinued, provided there is no history of high-grade cervical dysplasia or malignancy. However, for those who have undergone vaginoplasty involving tissue grafts, routine visual inspection of the neovagina during general pelvic examinations ensures early detection of tissue changes, inflammation, or suture complications.
Prostate Health and Lower Urinary Tract Surveillance
The prostate gland develops from embryonic tissue under the influence of androgens. Transgender women who undergo gender-affirming surgeries, including orchiectomy or vaginoplasty, retain their prostate gland. The prostate is not removed during standard vaginoplasty procedures, as doing so carries a high risk of urinary incontinence and nerve damage.
Because feminising hormone therapy lowers serum testosterone to castrate levels, the risk of developing prostate cancer in transgender women is significantly lower than in cisgender men. Estrogen therapy and anti-androgens typically cause the prostate gland to shrink. However, prostate cancer can still occur, and when it develops in a low-testosterone environment, it can present as a more aggressive form.
Prostate screening in transgender women requires careful interpretation:
- Prostate-specific antigen (PSA) levels are normally suppressed by estrogen therapy, meaning a PSA value within the typical range for cisgender men might actually represent an elevation in a transgender woman.
- Digital rectal examinations remain clinically useful, but for individuals who have undergone vaginoplasty, examination can also be performed via the anterior vaginal wall.
- Any symptoms such as dysuria, hematuria, decreased urinary stream, or pelvic pain warrant thorough urological evaluation.
Healthcare providers must ensure that transgender women understand that their prostate remains intact after surgery, dispelling common misconceptions that can lead to delayed reporting of urinary symptoms.
Bone Health and Endocrine Monitoring

Intersex individuals and transgender people undergoing gender-affirming hormone therapy require proactive bone health management, particularly if they experience periods of hypogonadism.
Transgender individuals who undergo gonadectomy (orchiectomy or oophorectomy) and subsequently discontinue or reduce hormone therapy are at a heightened risk for premature bone loss and osteoporosis. Similarly, intersex individuals who underwent surgical gonad removal in childhood or adolescence without adequate, timely hormone replacement therapy face significant long-term risks to bone integrity.
Dual-energy X-ray absorptiometry (DEXA) scans are recommended to assess baseline bone density in specific scenarios:
- Prior to initiating hormone therapy if history indicates prolonged periods of hypogonadism.
- At age 60 for individuals who have been on long-term hormone therapy.
- At an earlier age for individuals with history of surgical gonadectomy without continuous hormone replacement, or those with additional risk factors such as low body mass index, smoking, or chronic glucocorticoid use.
When interpreting DEXA scans, software calculates T-scores and Z-scores based on reference populations. Modern clinical guidance suggests comparing bone density results against reference data matching the individual's dominant circulating sex hormone over the preceding years, rather than relying strictly on legal sex markers.
